Found in one of the office rooms. This form seemed to be a way of recording how independent a paitent was, measuring which of the tasks listed they were capable of doing. We only found blank forms, which was a little disappointing.

_{Please note: Do not bother asking how to get into this building, as I am not going to say. Be aware that urban exploration of abandoned and damaged buildings is highly dangerous (due to broken flooring, glass, asbestos, lead paint etc).}

it makes me wonder how horrible and alone people must have felt in this place cos if you notice there isnt a slot for a name, just one for a number. going into these kind of places all you have left is your name and yet here they dont even grant you that, they take away the last peace of humanity you have left with you and replace it with a statistic.

If you look at the top of the page doesn't it look like it was a copy. I should know I copy stuff (the slips) for projects to reminded me what the project is about. And this place reminds my of a hopital in England on an island that is said to be haunted. I have seen videos of spirit chasers going into the hopital and it was haunted I know how to find out if there is a spirit near by that is how I could tell it was real.

It started closing in the early 1990's. It was totally shut down by 1994.
